Through this new partnership, KHA and ChartSpan will work together to identify providers that can benefit from CCM, a turnkey, value-based care and reimbursement program, and Annual Wellness Visit (AWV) services. These services improve patient outcomes and drive significant new value-based revenue to providers – an especially important achievement right now as many patients and clinics must postpone well-checkups and traditional sick visits. Leveraging a CCM program can improve patient engagement and health outcomes while providing a predictable source of reimbursement, or income, for clinics and practices.
ChartSpan can provide their services to Kentucky's critical access hospitals and clinics as well as standard fee-for-service (FFS) and specialty practices.
