IntellaTriage, the nation's leading provider of nurse-first hospice and home health triage solutions, has launched an innovative data integration with CommonSpirit Health at Home, one of the nation's largest nonprofit healthcare systems.
BRENTWOOD, Tenn. - ncarol.com -- IntellaTriage, the nation's leading provider of nurse-first hospice and home health triage solutions, and CommonSpirit Health at Home (CSHaH), one of the nation's largest nonprofit healthcare systems, went live with a secure integration between Homecare Homebase and IntellaTriage's triage platform, IntellaHub. Partners since 2022, the companies recognized that a deeper integration of their systems could greatly enhance their shared mission of improving patient and caregiver experiences. Home health and hospice electronic medical records (EMR) often lack interoperability, hindering efficient information exchange and patient care in the field. By breaking down these silos, both companies anticipate improved workflows, eliminated redundancies, and enhanced experiences for patients and field nurses.
Since launching its partnership with IntellaTriage, CommonSpirit Health at Home has witnessed significant improvements in patient outcomes and staff retention, attributed to the partnership and other strategic initiatives. The two organizations have continued to deepen their partnership, with the integration between the two organizations primary record keeping systems as a natural next step to future collaboration.
In 2023, IntellaTriage officially launched IntellaHub, a proprietary triage platform with features such as advanced analytics and benchmarking, improved communication between field and triage teams, and best-in-class triage algorithms. The integration with Homecare Homebase marks a major milestone for IntellaHub on the path to ensuring a completely seamless experience between daytime and after-hours care for patients and their families.

Daniel Reese, CEO of IntellaTriage, expressed his enthusiasm about the partnership, stating, "We value our partnership with CommonSpirit Health at Home and are proud to contribute to their innovative No More On-Call home health triage model, which has significantly improved patient outcomes and operational efficiency. Choosing them as our partner for this initial integration was an obvious decision." Reese also highlighted the company's long-term goals, including IntellaHub integration with other major EMRs across the industry, as part of IntellaTriage's commitment to technology-supported, patient-centric care.
Michael Brents, Chief Clinical Officer/ VP, Informatics at CommonSpirit Health at Home, spearheaded the project on the provider side and was a major proponent of moving it forward. "We are excited about the new technology IntellaTriage has implemented to enhance our security measures and support patient care. Our results since partnering with IntellaTriage have demonstrated improvements in patient care metrics and care team well-being. This integration with our electronic medical records will further streamline operations and enhance financial performance for CommonSpirit, leading to a direct positive impact on our nurses and patients."
